Choosinganinterventionforyourchild Nowthatyouhavechosentheinterventionforyourchildwithautism,understandingthegoalsforeachinterventionisthenextstep.Theinterventionthatyouchooseforyourchildshouldhaveveryclear: goalsandobjectives methodtorecordofyourchild’sprogress systemforgeneralisingtheskillstootherenvironments processforcommunicatingtheprogresswithyou. TheEarlyStartDenverModel(ESDM)isanearlyinterventionmodelforyoungchildrenwithautism.Ittargetsthekeyfeaturesofautism,and involvesintensiveteachinginplay-basedsessions.BuiltintotheESDMisacomprehensiveprocesstodevelopgoalsandmonitortoeachchild’sprogress.Thisleadstoyourchildbeingabletoindependentlyusetheirnewskillsacrossanumberofenvironments. Eachchild’sprogressintheESDMismonitoredinthefollowingways Thecurriculumchecklist Thefirststepinmonitoringgoalsiscompletingtheassessment.Thishelpsustounderstand: Thethingsthatyourchilddoeswell Theareasthatchallengeyourchild Thesequenceofskillstoteachyourchildineachdevelopmentalarea WeusetheEarlyStartDenverModel(ESDM)CurriculumChecklistasourassessment.Thisisaplaybasedassessmentwherethetherapistusestoysandsocialactivitiestoassessyourchild.Thetherapistfollowsthechild’sinterestandpreferencefortoys.Whenthechildapproachesatoythenthetherapistwillplayandengagewiththechildastheyscorethechild.TheEarlyStartDenverModelCurriculumChecklisthas 480curriculumitemsacrossfourlevels. receptivelanguage expressivelanguage imitation play jointattention socialskills cognition finemotorskills grossmotorskills behaviour personalindependence Developthegoals FromthecurriculumchecklistthecertifiedESDMtherapistcreatesyourchild’sfirstsetofgoalsandobjectives.Wechoose3-4goalsfromeachdevelopmentalarea.Wechoosethesegoalsstartingfromthepointwherethechildneedsmoreskillsineachdevelopmentalarea.SomechildrenhavegoalsstartinginLevel1,whileotherchildrenhavegoalsthatstartinLevel2orLevel3. Writetheteachingsteps Thetherapistwritesteachingstepsforeachgoal.Thismeansthateverygoalthatischosenforyourchildhasindividualisedteachingstepstoteachyourchild.Itisalsousedtomonitortheiracquisitionofthatgoal.Theteachingstepsarewrittentoprogressyourchildfromtheircurrentlevelofability,tousingtheskillindependentlyacrossanumberofsettings. Codeeverysession TheESDMtherapistkeepsarecordofeveryactivityineverysession.Thedatarecordsthelevelofpromptingthatisneededtoteachyourchildonthatday. Anumberofdifferentpromptsareusedinteaching: fullprompt–completephysicalassistancetolearnthegoal verbalprompt–talkingtothechildtogivethemverbalassistancetoachievethegoal gesturalprompt–somephysicalassistanceoragesture toachievethegoal acombinationofverbalandgesturalprompts Ourgoalisforyourchildtoindependentlyuseordemonstratetheskillwithnoprompting. Feedbacktoparentsaftereachsession ThecomprehensivenatureofthegoalsandteachingstepsintheESDMmeanthatthetherapistcanclearly,andeasily,showtheparentsthechild’sprogressateverysession. Generalisegoalstootherenvironments Onceyourchildhasshownthattheyindependentlyusetheskill80%ofthetime,westartthe‘out-of-clinic’generalisationsteps.Wewanttomakesurethatyourchildusestheskillindependentlyathomeanddaycare.Ifwewereteachingyourchildtousearingstackerthenweneedtomakesurehedoesthisskill: intheclinic3daysinarow athomethreedaysinarow,or atdaycarethreedaysinarow Whenyourchilddoesthis,thegoalsare‘generalised’.Whengoalsaregeneralisedacrossenvironments,andwithmorepeople,thataremorelikelytobemaintainedandusedbythechild.Thereisnopointteachingachiildaskilliftheyonlyuseitintheclinicsituation. Changetherapistsforasessionevery8-10weeks Childrenbecomeusedtotheirregulartherapists.Theyknowthemwellandhaveafamiliarroutinewiththeirtherapist.Every8-10weeksyourchildwilldoasessionwithadifferenttherapist.Ifthescoresforthatsessionareconsistentwiththeothertherapistscodes,thenthechildisusingtheskillsconsistently.Ifthescoresvary,wewilllookatwaystohelpyourchildbemoreconsistentwithavarietyofpeople.Thismakesadifferenceingeneralisingtheskilltoalltheotherplacesthatyourchildisapartof. Reviewgoalsasprogressismade Assoonasagoalisgeneralised,anewgoalisaddedtothelist.Wewriteteachingstepsforthatgoal.Onceagain,theteachingstepsareindividualisedforyourchild.
